About the Covid-19 Vaccination Process

Read Covid-19 Vaccine Facts, FAQs and Process. You will require two Covid-19 injections. The second injection is 3-8 weeks later to boost immunity further. If you already had 2 injections and if it has been 6 months, take the booster. There is a second booster for those 50 and older or for those who are immunocompromised (dose #4) given 3 months from the third dose (booster #1) that is recommended.

Even if there is a lack of Covid-19 vaccine data on a large scale over a long period of time, there is no stated reason for not getting a Covid-19 vaccine. In some cases, caution should be taken and timing may have to be adjusted when getting the vaccine. You may have to take additional current medications or avoid taking current medications either before or after the Covid-19 vaccine.

We will review your information when you request a vaccine and submit it to order the Covid-19 vaccine from California State. We pick up from the county local public health department on a weekly basis.

For your convenience, we may send you back a link to register as a patient on our Patient Portal called Patient Ally where you can view your health records and lab results, make or reschedule appointments, request Rx refills, view your account statement, pay your bill online and send non-urgent messages online 24×7 with privacy.

STEP 1: Reserve Vaccine and Book An Appointment online or call us at (408) 945-0300.

We know your time is precious. We are planning for dedicated vaccine days in the clinic to streamline the vaccination process and get you vaccinated within 20 minutes. You may be asked to wait for 10 to 20 minutes in your car to observe any adverse reaction.

STEP 2: Come and Get your 1st Covid-19 Vaccine Injection. Make sure to bring the Pre-Vaccination Checklist & Attestation.

Come a few minutes earlier and make sure to wear a mask, keep social distance and wash or sanitize your hands. Dress comfortably so that your arms can be very easily accessed for injection as we may not be able to give you gowns or privacy to take off your clothes, etc. For your safety, the less time you spend in the clinic, the better it is for you and others. You want to get in and out quickly.

Make sure to bring with you a printed copy of your Pre-Vaccination Checklist with updated info, your valid Photo ID such as a CA Driver License or Passport and your Insurance card (if uninsured, SSN). In our clinic, the staff will check you in, verify your stated health status that you are not sick and upon receiving these documents we will give you a vaccine.

We will not be able to give any medical advice, consult or health check during this visit. If you need to consult a physician, you are encouraged to make a separate appointment via TeleMed or In-Clinic either prior to and/or after the vaccination.

After getting a Covid-19 vaccine shot, you are advised to wait in your car for 10-20 minutes as a precaution in case of an unlikely medical event. If an allergic or adverse reaction occurs, you can receive a doctor’s attention immediately or an ambulance can be called as needed.

After your first injection, we will schedule or you can schedule yourself online (on Patient Ally Patient Portal) your second Covid-19 injection appointment for a later date (about 3 weeks for Pfizer and 4 weeks for Moderna as stated in their VIS).

Your appointment will be confirmed and we will make sure a Covid-19 vaccine is in stock for you and is available when you come.

STEP 3: Come and Get your Second Covid-19 Injection.

After we have determined the vaccine availability & we have confirmed your 2nd appointment, you will get your 2nd injection appointment.

Again, wear a mask and keep social distance similar to the first injection to boost your immunity level against Covid-19. Get your injection. Wait for 10 minutes in your car.

You are DONE! Your immunity will develop fully in a few weeks after the second vaccination injection against this highly communicable and dreadful disease.

Remember to get your boosters as advised by your healthcare provider or as needed.

After Vaccination, you will be protected and it will help you not get a Covid-19 infection in the future and spread it to others.

We will also issue you a proof of Covid-19 Vaccination Record Card that may be needed by you for travel, employment, school or college. After we report your Covid-19 vaccination data to the California Immunization Registry (CAIR), you will be able to download a week later your Digital proof of vaccine on your smartphone or laptop.

Please note that all your medical and vaccination history can be accessed on our Patient Ally patient portal online 24×7.

We are pleased that after reviewing the Covid-19 Vaccine Info (VIS), you want to reserve your Covid-19 vaccine with our clinic.

Vaccination will be provided as per priority set by the state of CA and CDC and is on a first come first serve basis. Existing patients with a vaccine reservation will be given priority. New patients are welcome. If you are not currently our patient, you can book a TeleMed appointment for a well visit, annual physical, preventive care or any disease consult and become our patient.

If you have any health concerns or potential contraindication conditions, allergies, or need more info, please book an appointment either by TeleMed video or in-clinic with our physicians.

In the meantime, till you get vaccinated, stay safe, follow the CDC recommendations of washing hands, masking, social distancing etc. and get preventive care, annual physical and flu shot, etc.

We expect you to come for your scheduled appointment as we have reserved the vaccine and staff to take care of you; otherwise, we could have given the vaccine to someone else in need. If you miss the appointment, you will have to reserve it again and wait for your turn. You may be charged for a missed appointment, if 48 hour notice is not given.

If it is an emergency, dial 911 or go to the nearest emergency room.

This vaccine reservation request is not a confirmed appointment until you are enrolled in our Patient Ally EHR Patient portal and you have confirmed the appointment and we have received the allocated vaccine for your phase.

If you have any of the symptoms such as fever, cough, cold, headache, loss of smell, feeling sick or fatigued you will have to wait to get a vaccine until you are symptom free and have waited for an additional 15 days symptom free, or have gotten clearance from your physician. If you have any of the 3 Contraindications listed in the CDC Pre-Vaccination Checklist for Covid-19 vaccine, you can be given a vaccine only with written authorization from your physician.

IMPORANT NOTE:

  1. Items 3 to 5 are Contraindications to these Vaccines. If you checked Yes to any of Item 3 or 4 or 5 in this Pre-Check list below, we will NOT give you a vaccine. We will not reserve a vaccine for a patient with this medical history. You are advised NOT to take a Covid-19 vaccine until fully cleared by your physician.
  2. If you checked Yes to any of these 4 items from 6 to 9 in this Pre-Check list below or are feeling sick or have, you will be able to take the vaccine after a waiting period and consult by the physician.
  3. If you checked Item 10 or breast feeding in item 11, we can give you a vaccine by taking appropriate precautions. Let us know ahead of time and preferably book a consult with our physician. COVID-19 vaccines were tested in large clinical trials to make sure they meet safety standards. Many people were recruited to participate in these trials to see how the vaccine offered protection to people of different ages, races, and ethnicities, as well as those with different medical conditions. What are Possible Side Effects of the Covid-19 Vaccine?

You may experience some side effects after receiving the vaccine, which are normal signs that your body is building protection. Most side effects are mild to moderate, including but not limited to chills, tiredness, headache, or pain with swelling and redness at the injection site. However, a small number of people may experience severe side effects that affect their ability to perform daily activities, but this should resolve within a few days. We ask you to stay in our clinic for a minimum of 15 minutes to make sure there is no acute potentially life-threatening allergic reaction.

If you have a history of allergy, cancer, diabetes, kidney or heart disease or any serious underlying health conditions, you are advised to consult your physician. You can book a TeleMed consult with our physician in case you do not have access to your physician.

Are There Any Long Term Side Effects or Complications Due to Covid-19 Vaccination?

COVID-19 vaccines are being tested in large clinical trials to assess their safety. However, it does take time, and more people getting vaccinated before we learn about very rare or long-term side effects. That is why safety monitoring will continue. The CDC has an independent group of experts that reviews all the safety data as it comes in and provides regular safety updates. If a safety issue is detected, immediate action will take place to determine if the issue is related to the COVID-19 vaccine and determine the best course of action.

How Many Covid-19 Vaccine Shots are Needed and Why?

Nearly all COVID-19 vaccines being studied in the United States require two shots. The first shot starts building protection, but everyone has to come back a few weeks later for the second one to get the most protection the vaccine can offer. The Pfizer vaccine is repeated 21 days later while the Moderna second shot is given 28 days later. A 3rd injection or a booster shot is required after 6 months to boost immunity. For children between ages 5 to 11 years, two shots are required and booster shots are not presently required.

Is it Better to Get Natural Immunity Rather Than Immunity From Vaccines?

Both the disease and the vaccine are new. We don’t know how long protection lasts for those who get infected vs those who are vaccinated. What we do know is that COVID-19 has the potential to cause serious illness with increased risk for mortality. If you get COVID-19, you are also at risk for giving it to loved ones who may get very sick. Getting a COVID-19 vaccine is a safer choice.

What if I Already Had a Covid-19 Infection. Should I Get Vaccinated?

Yes, you should get the COVID-19 vaccine. Due to the severe health risks associated with COVID-19 and the fact that re-infection is possible, vaccination for COVID-19 should be considered regardless of whether you already had a COVID-19 infection.

However, anyone currently infected with COVID-19 and/or with active symptoms should wait to get vaccinated until after their illness has resolved and after they have discontinued isolation.

It is advised to get a vaccine even if you were infected with Covid-19 and have built some immunity.

COVID-19 vaccination should be offered to you regardless of whether you already had a COVID-19 infection. You should not be required to have an antibody test before you are vaccinated. Only a fraction of people show Covid-19 symptoms after infection.

Can I Take a Covid-19 Vaccine Before Travel? What Can I Do?

If you have less than four weeks before your trip, a consultation may be of benefit, depending on how long you are traveling and where you are going.

The Covid-19 vaccine needs two shots 3 to 4 weeks apart. It is better to travel a week after your second shot. It is also a good practice to travel a week after taking the booster shot to allow time for general side effects such as tiredness, headache etc. to go away and immunity to develop.

You may need additional vaccines depending upon your destination. Most vaccines will not be effective for two to three weeks unless you have had prior immunization with the specific vaccine. However, vaccination prior to exposure is better than no protection. Older patients may be a candidate for immunoglobulin for prevention against Hepatitis A. Always take the flu vaccine annually, particularly before travel, events or gatherings.

Prescriptions also may be needed for malaria protection and travelers’ diarrhea treatment. If your trip is longer than three weeks from the time of the vaccine administration, then vaccines will be of benefit. The advice on non-prescription health prevention measures may be especially beneficial for those not receiving vaccinations.

What Information Should I Bring Before My Consult Appointment?

Please bring a form of identification and your insurance card (if insured). You may pre-fill the form or on our patient portal or website, to save you time. If you reserve the vaccine, you will be invited to join our Patient Ally portal where you can enter your demographics and past medical history.

You should also be prepared to detail your health history, current medications, immunizations, vaccines and history of environmental/food/chemical allergies to make sure you can get a Covid-19 vaccine safely.

What is the Difference Between Required and Recommended Vaccinations?

In general, “Required vaccinations” are for the protection of the visited country against the disease you might bring into their country. “Recommended vaccines” are ones that you should have for your own health protection. Yellow Fever Vaccine is required for travel to some countries whose mosquitoes could acquire the disease from you, if you are not protected from this disease; however, you should also consider other vaccines for your protection such as typhoid, Hepatitis A, and others in countries that require only Yellow Fever vaccination.

During your consult, the physician will recommend the best plans based on your health history, travel plan, cost and other concerns.

Are There Side Effects From the Vaccines and Medications?

Any medication taken has some side effects. During your consult, the physician will address any of these concerns and weigh benefits vs side effects and present you with a choice.

Most vaccines have minimal side effects beyond a slightly sore arm at the injection site. Persons being vaccinated will receive a vaccine information sheet (VIS), which describes the benefits, and possible common and uncommon side effects. You need to inform us of any prior history of adverse vaccine reactions.

MEDICAL CONDITIONS OF CONCERN

If you have one or more conditions in your medical history, you must consult your physician prior to getting a Covid-19 vaccine. We will not be able to vaccinate you without prior authorization from your physician.

  • You have previously received a Covid-19 Vaccine and had a severe allergic reaction (anaphylaxis) after receiving the Covid-19 Vaccine
  • Have a Bleeding Disorder or I am taking a blood thinner
  • Have a Fever
  • Feeling sick
  • Pregnant or could become pregnant in the next few weeks
  • Breast feeding
  • Have Obesity (BMI 30 or higher but less than 40)
  • I have one or more of these conditions: Cancer, Chronic Kidney Disease, COPD, Heart Condition (heart failure, coronary artery disease or cardiomyopathies), Severe Obesity > 40 BMI, Sickle Cell Disease, Immunocompromised state (weakened immune system) from solid organ transplant.
  • Smoking
  • Type 2 Diabetes mellitus
  • Have received another vaccine in the last 14 days
  • Had a severe allergic reaction (anaphylaxis) to another vaccine or any other injectable medications
  • Have weakened immune system caused by Cancer, HIV infection or I am taking immunosuppressive drugs or therapies
  • Have been treated before with epinephrine or EpiPen or had to go to the hospital for that
  • Have received passive antibody therapy (monoclonal antibodies or convalescent serum) as treatment for Covid-19
  • Had a positive test for covid-19 or were told by a doctor that you have Covid-19

CONTRAINDICATIONS

  • Severe allergic reaction (e.g., anaphylaxis) or immediate allergic reaction after a previous dose of an mRNA COVID-19 vaccine or any of its components

Components include: Polyethylene glycol, Tromethamine, Acetic Acid, Sodium Acetate, Sucrose

History of any immediate allergic reaction to above components in any form or in a vaccine or injectable therapy. If so, please discuss with your physician about the allergen trigger and response before taking a covid-19 vaccine.

You are advised not to get a vaccine from us and consult your physician if you have any of the above contraindications.

Why Do I Need a Covid-19 Vaccine if I Am Wearing a Mask and Social Distancing?

Stopping a pandemic requires using all the tools available. Vaccines work with your immune system so your body will be ready to fight the virus if you are exposed.
